Download all Application Forms (See PDF files below) or obtain an Application Packet from the school office Return completed forms to the school office ATT: Admissions Fill out the Transcript Record Form and bring it to the school your son/daughter currently attends As necessary, ask for a Financial Aid Form from the school office NOTE: During the application process the school may request a letter of reference from a teacher or school official Students entering the school for the first time are required to provide the following: A copy of the child’s birth certificate All current immunization records. All new students must have a recent physical examination, documented within 12 months prior to entering school. A copy of the child’s Individual Education Plan (if applicable)
